I fixed it, for OpenNebula 4.0 and 4.2.

I just found there has been a patch for this bug in #2106
There is a little difference betwen mine and #2106. I get image source which will be attached from xpath $DISK_XPATH/SOURCE instead of an explicit "one/one-$IMAGE_ID" string. In some cases, we use a different Ceph pool name rather than default 'one'.
